The Georgia Pediatric Program (GAPP) helps eligible children under age 21 receive medically necessary nursing and personal support at home. If your child has Georgia Medicaid and needs ongoing care, we can help you understand the program, coordinate with their physician and take the next step. Medicaid-approved GAPP services are provided at no cost to eligible families.

His background includes commercial health insurance underwriting, Medicaid enrollment and provider assistance across multiple states, and quality assurance. At Nobility, James guides administrative operations and intake coordination with a strong emphasis on accuracy, efficient processes, and clear communication. He aims to help families understand program requirements, navigate paperwork, and make informed decisions about care at home.

Diana Mujahid, RN

Diana Mujahid, RN, brings experience in pediatric nursing supervision, home health, wound care and risk management. As RN Supervisor, she guides care planning, supports the nursing team and helps families understand their child’s care. Her focus is safe care at home and clear communication with the people who know each child best.

GAPP: Answers for Parents

Understand your options before you begin. Nobility helps families navigate pediatric home care, physician documentation and the GAPP intake process.

Who may qualify for GAPP?

GAPP serves Georgia Medicaid-eligible children under age 21 who are medically fragile and need medically necessary care at home. Eligibility, services and hours are reviewed through the program’s authorization process. Contact us to discuss your child’s needs and next steps.

What care can my child receive at home?

Depending on the approved care plan, services may include skilled nursing and personal support. Nursing can include medication and treatment support, respiratory care, feeding support and clinical monitoring. Personal support can help with daily activities such as bathing, dressing, positioning and mobility.

Does my child’s physician need to be involved?

Yes. We coordinate with your child’s physician for the required orders and care documentation. Our nursing team helps assess needs and prepare the information for the authorization request.

What does GAPP cost my family?

Authorized GAPP services for eligible children are funded through Georgia Medicaid and provided at no cost to the family. Any services outside the approved GAPP benefit should be discussed separately before care begins.

What happens after I contact Nobility?

We talk with you about your child’s needs and location, explain the paperwork, coordinate the nursing assessment and physician documentation, and submit the request for review. After authorization, we arrange care according to the approved plan and staffing availability.

How long does intake take?

Timing varies with the required documentation, physician response, authorization review and staffing. We explain the next steps and keep you informed. An inquiry does not guarantee approval, a specific number of hours or an immediate start date.

Can a family caregiver take part in my child's care?

The GAPP Family Care Option may allow an eligible family caregiver to become an agency employee for authorized personal support. Employment, CPR, training, supervision and documentation requirements apply. Read the Family Care Option details before deciding whether to pursue it.
